Why do forehead wrinkles form?

Forehead wrinkles are the horizontal lines that run across the forehead, deepening when you raise your eyebrows. In younger skin, they appear only with movement. Over time, as skin loses collagen and elasticity, they remain visible even when the face is at rest. These are sometimes called dynamic lines and static lines, and the difference shapes treatment.

Most people assume forehead lines are simply a sign of expression or age. For many, that is true. But a surprising number of patients have forehead lines because their forehead is working harder than it should, lifting the brows all day to keep heavy eyelids out of the way. When this is the case, the lines are a symptom, and treating the forehead alone can make things worse.

The anatomy of the forehead

The forehead is moved mainly by the frontalis, a broad, thin muscle that runs vertically from the scalp to the brows. It is the only muscle that lifts the eyebrows. Each time it contracts, the skin above it folds into horizontal lines. Opposing it are the brow depressors: the corrugator, which pulls the brows together and creates vertical frown lines; the procerus, which pulls the inner brow down and creates a line across the bridge of the nose; and the outer part of the orbicularis oculi, the ring-shaped eye muscle, which draws the brow tail down.

The frontalis also acts as a reserve lifting system for the eyes. When the upper eyelid opens weakly, a condition called ptosis, or when excess eyelid skin or a descended brow crowds the lid, the brain recruits the frontalis to raise the brows and clear the field of vision. Over years, that constant effort etches deep lines. Understanding this relationship is central to good treatment.

How forehead wrinkles are assessed

Dr. Kim Jun-young, representative surgeon, begins by observing your forehead at rest and in expression. He then asks you to close your eyes and relax, and gently holds the brows in place while you open them. If the brows immediately rise and the lines return, or if the eyelids feel heavy without the brows' help, the forehead is likely compensating for the eyes.

He measures eyelid opening, skin excess on the upper lids and brow position. He also assesses skin thickness, the depth of static lines, forehead contour and any previous injectable treatments. Endoscopic forehead lift

When the brows have descended and the forehead is lined, an endoscopic forehead lift repositions the brows through small incisions behind the hairline and can weaken overactive depressor muscles.

How to choose the right approach

If your lines are mainly from expression and your eyelids open well, Botox and skin treatments are often enough. If you notice your brows rising all day, your eyelids feel heavy, or Botox has made your eyes feel heavier in the past, the eyes likely need attention first. If the brows themselves sit low and the upper face looks heavy overall, a forehead lift may give the most coherent result.

Can Botox make my eyes look heavier?

Yes, in some people. If your frontalis muscle is lifting your brows to compensate for heavy eyelids or a low brow, relaxing it with Botox removes that support, so the brows and eyelids can sit lower. Why do I raise my eyebrows all the time?
