Filler at its most useful is quiet: a small amount, placed with care, that brings back a curve or a shadow you had forgotten you had.
Dermal filler is an injectable gel, most commonly hyaluronic acid, placed beneath the skin to restore lost volume, soften folds and refine contours such as the temples, cheeks, chin, nose and lips. Treatment takes about 15 to 30 minutes. Results are visible immediately and typically last around six to eighteen months, depending on the product and area.

What is dermal filler?
Dermal filler is an injectable gel used to add volume or shape beneath the skin. At Will Be, the fillers used are mainly based on hyaluronic acid (HA), a sugar molecule that occurs naturally in skin and binds water. Cross-linking, a chemical process that links HA chains together, makes the gel firmer and slows the body's breakdown of it, so it lasts months rather than days.
Different HA fillers vary in firmness and spread. Firmer products support structure, for example at the chin or along the cheekbone; softer products suit delicate areas such as the lips or under the eyes. Choosing the right product for each layer is part of the treatment.
Who is dermal filler suitable for?
Filler may suit you if you have:
- hollow temples or flattened cheeks from facial volume loss;
- deepening folds, such as nasolabial folds;
- a recessed chin or a soft jawline you would like to define;
- a low nasal bridge you want to try raising without surgery;
- a wish to test a change before committing to a surgical option.
Filler is not suitable during pregnancy or breastfeeding, with active skin infection in the area, or with certain autoimmune conditions without careful assessment. It is also not the answer to significant sagging: adding volume to heavy, descended tissue can make the face look larger and heavier. In those cases lifting or fat grafting may be more appropriate.
How Dr. Kim approaches filler
At Will Be, injectable treatments are assessed and performed by Dr. Kim Jun-young himself, the same surgeon who performs the clinic's operations. There is no hand-off to another injector. That matters because the judgement behind a good injectable result is surgical judgement: understanding the layers of the face, where vessels and nerves run, and what a treatment can and cannot achieve compared with surgery. When an injectable is not the right answer, you will be told so.
Dr. Kim assesses your face at rest and in motion, and asks what you notice in the mirror. Filler is placed to restore your own proportions, not to follow a trend. He generally prefers modest volumes in the right layer to large volumes that distort expression, and he will suggest reviewing the result after two weeks before adding more.
Technique and anatomy
The face has distinct layers: skin, superficial fat, the SMAS (a fibrous and muscular support layer), deep fat compartments and bone with its covering membrane, the periosteum. Volume loss with age happens in several of these at once, especially in the deep fat compartments of the midface and temples, and in bone itself.
Filler is therefore placed at the depth where volume has been lost. Structural support is often placed deep, close to bone, using a needle or a blunt cannula; softer refinements are placed more superficially. Blunt cannulas can be used in certain areas because they are less likely to pierce vessels. Injection is slow, in small amounts, with constant attention to how the tissue responds.
How long does dermal filler last?
Honest answer: it depends. Most HA fillers last around six to eighteen months. Areas with more movement, such as the lips, tend to break filler down faster; deep, still areas such as the temples or chin may hold it longer. Individual metabolism, product choice and volume also play a part. The effect fades gradually rather than disappearing overnight.
A practical advantage of HA filler is reversibility. If a result is uneven or a complication arises, an enzyme called hyaluronidase can dissolve it. This is a key reason Will Be favors HA products.
Before your appointment
Preparing for dermal filler is simple, but a few steps help. Tell Dr. Kim about your medical history, allergies, medications and any previous injectables or surgery in the area, including the product names if you know them. Blood thinners, aspirin, some anti-inflammatories, fish oil, vitamin E and certain supplements can increase bruising; do not stop prescribed medication without medical advice, but mention everything. If you have an important event, plan treatment at least two weeks beforehand so that any swelling or bruising has settled and the result can be reviewed. Arrive without makeup on the treatment area if possible.
What happens during treatment and afterward?
- Before: numbing cream is applied; many fillers also contain lidocaine, a local anesthetic.
- During: injections take about 15 to 30 minutes.
- Afterward: mild swelling, redness or small bruises are common for a few days. Avoid pressing or massaging treated areas, strenuous exercise, saunas and alcohol for about 24 to 48 hours.
- Two weeks: the filler settles and integrates. This is a good time to review the result.
Risks and how they are minimised
Common side effects include swelling, bruising, tenderness and small lumps, which usually settle. Less commonly, infection, delayed inflammatory nodules or filler migration can occur. The most serious risk is vascular occlusion, where filler enters or compresses a blood vessel. This can cause skin injury and, rarely, vision loss. The nose, glabella (between the brows) and nasolabial region require particular care.
Dr. Kim minimises these risks through detailed knowledge of facial vascular anatomy, careful choice of depth, the use of blunt cannulas where appropriate, slow injection of small amounts, and close observation. Hyaluronidase is kept on hand. Contact the clinic immediately if you notice severe pain, blanching, dusky skin or any change in vision after treatment.
Filler, fat or surgery?

Treatment during a visit to Korea
Filler requires little downtime, so many international patients have it on a short trip of one to three days, which allows a quick check the next day if needed. Flying soon after is generally acceptable, though swelling can be slightly more noticeable after a long flight. If you are also planning surgery during the same trip, Dr. Kim will advise on sequencing, since injectables are usually kept apart from areas that will swell after an operation. Can filler be dissolved?

What should I avoid after filler?
For about 24 to 48 hours, avoid pressing or massaging treated areas, strenuous exercise, saunas, very hot environments and alcohol. Sleep on your back if possible for the first night. Contact the clinic urgently if you notice severe pain, pale or dusky skin, or any change in vision.
