What are sagging jowls?
Sagging jowls are the soft pockets of tissue that form along and below the jawline, on either side of the chin. They blur the sharp line between the face and neck, giving the lower face a squarer or heavier look. A shallow groove often appears in front of each jowl, and when viewed from the side, the angle of the jaw may seem less defined.
Jowls typically begin in the forties, earlier in some people and later in others, depending on genetics, skin quality, bone structure and weight history. They often appear alongside deeper nasolabial folds and a looser neck. For many patients, the jawline is the first place they notice that their face no longer matches how they feel. The anatomy behind jowl formation
The face is built in layers. Beneath the skin and fat lies the SMAS, the superficial musculoaponeurotic system: a supportive sheet of connective tissue and muscle fibers that envelops the face and continues into the platysma, the thin muscle of the neck. The SMAS is anchored to the underlying bone by fibrous retaining ligaments, which act like the pins in a tailored garment.
With age, these ligaments and the SMAS loosen. Fat that once sat high on the cheek and along the jaw slides downward until it meets a firm anchor near the jaw, the mandibular ligament. Tissue collects above and behind this point, forming the jowl, while the ligament holds the skin in front of it, creating the groove. Meanwhile, the jawbone gradually loses some volume, the skin thins and loses elasticity, and the platysma may separate into bands. All these changes combine to soften the jawline.
How sagging jowls are assessed

A natural lift, not a pulled look
An overly tight or wind-swept appearance usually results from pulling the skin rather than repositioning the deeper layers. Can sagging jowls be treated without surgery?
Early jowling can be softened with thread lifts and collagen-stimulating treatments, which lift and firm the tissues modestly with little downtime. However, they cannot reposition significantly descended SMAS or remove excess skin. When jowls are moderate or advanced, a mini facelift or facelift usually gives a clearer and longer-lasting improvement.
How long does a thread lift last for jowls?
The visible lift from threads typically lasts several months to a year or more, depending on the thread type, your tissue and lifestyle.
